As GLP-1 drugs move beyond their origins in diabetes, research is splitting into distinct new territories. Two major shifts define this evolution: a move toward long-term cardiovascular and renal protection and a new focus on managing the drugs' direct impact on gastric function.

The GLP-1 drugs are everywhere this week — a CBS investigation found clinics openly prescribing retatrutide, an experimental Eli Lilly weight-loss drug that hasn't been approved, while a gray market sells research vials to people who can't wait. The noise is all about weight. We went looking for what the science is actually chasing.

We pulled 1,500 of the most recently registered clinical trials of the GLP-1 and incretin family — semaglutide, tirzepatide, liraglutide, retatrutide and their kin — from ClinicalTrials.gov, the U.S. government's public registry. Each [[document]] is one trial, tagged with the condition it set out to treat. We let the collection sort itself and read the [[map]].

The widest split in the data is a story about escape. At one pole sit the drugs' origins — weight loss, obesity, the mechanics of stomach emptying, the metabolic work these molecules were built for. At the far opposite pole is where the research has traveled: the heart and the kidneys — trials for heart failure, coronary disease, kidney failure, cardiovascular prevention. The single biggest thing distinguishing one trial from another is no longer the dose or the formulation. It's how far the drug has moved from the waistline toward the organs.

And the corpus doesn't stop there. A second [[bearing]] picks out the trials studying the drugs' direct effect on the gut — delayed gastric emptying, and the surgical-safety questions that come with it. A third tracks the quiet engineering race to turn an injection into a pill. Underneath all of it, a long tail: 852 distinct conditions in all, with single trials reaching into sleep apnea, alcohol use disorder, even cognition. A diabetes drug becoming, trial by trial, a general-purpose medicine.
